Change of Name


If your surname has changed subsequent to marriage and you want a new passport or an additional booklet in your married name you will need to fill out the form meant for a new passport along with four recent passport-size photographs. You also need to enclose your current passport, your original marriage certificate with a photocopy and your spouse's passport. The fee for this service is �27.00 if you want an ordinary, 36 page, passport or �34.00 if you want a jumbo, 60 page passport. The fee for additional booklet is �14.00 if you want an ordinary 36 page additional booklet or �17.00 if you want a jumbo 60 page additional booklet.

In case you do not want a new passport, you can also apply for an observation regarding your change of surname to be entered in your existing passport. For this you need to fill out the form meant for Miscellaneous Services and the fee is only �7.00. The documentary requirements are the same as those mentioned above.

In case you wish to change your first name/surname or both your first name and surname, you will be required to place advertisements to that effect in a prominent daily newspaper of the area of your residence in the UK and a prominent daily newspaper of the area of your permanent address. You would be required to submit these advertisements (newspaper page) along with a sworn affidavit from a Notary Public in the prescribed format.
