If your appearance has changed significantly since your last passport was made, you can apply for a fresh passport or an additional booklet with a more recent photograph. You will need to fill out the form meant for a new passport along with four recent passport-size photographs. You also need to enclose your current passport, a sworn affidavit in the prescribed format from a Notary Public. The fee for this service is �27.00 if you want an ordinary, 36 page passport or �34.00 if you want a jumbo, 60 page passport. The fee for additional booklets is �14.00 if you want an ordinary 36 page additional booklet or �17.00 if you want a jumbo 60 page additional booklet.
