You also need to enclose a duly completed 'L' Form, provide a Police Report regarding lost or stolen passport, and a sworn affidavit in the prescribed format. The affidavit can be sworn at the High Commission itself for an additional fee of 8.00 pounds and you do not need to go to a notary public provided you are able to furnish documentation to support your above mentioned status. You may also provide a photocopy of your lost passport/expired previous passport, or any other document proving your Indian nationality.
